American CRNA Magazine Online and its electronic newsletters are owned by American CRNA Magazine and contain information, magazine content, and hypertext links to information of interest to our subscribers and the general public. The material and hypertext links provided on this Web site and in our e-mail newsletters are offered in good faith and for informational purposes only. American CRNA Magazine assumes no responsibility for any material on our site that may be accessed through any hyperlink presented on our site or in an e-newsletter or for any unauthorized uses of these links or of our material.

With the exception of those items denoted "public domain," all materials presented on American CRNA Magazine Online’s Web site or in a newsletter offered on the site are protected by international trademark and copyright laws and must only be used for personal, non-commercial purposes. II. American CRNA Magazine Privacy Policy

Information Collection

American CRNA Magazine is the sole owner of the information collected on AmericanCRNA.com. We will not sell, share, or rent this information to others in ways different from what is disclosed in this statement. American CRNA Magazine collects information from our visitors at several different points on the American CRNA Magazine site. In order to provide our visitors the greatest level of comfort on the American CRNA Magazine site, we do not collect personally identifiable information until we have notified the site visitor of how the information will be used. We do not share personally identifiable information collected on the American CRNA Magazine site with third parties without the visitor’s prior consent.

Registration

In order to use personalized services on the American CRNA Magazine site, a visitor will be asked to fill out a registration form. During registration the visitor is required to give his or her contact information (such as name and e-mail address). This information is used to maintain a list of authorized visitors and may be used to contact the visitor about the services on our site for which he or she has expressed interest. We require a visitor’s American CRNA Magazine registration ID (email address) to provide services such as access to Members Only information.

We request information from the visitor on order forms for products and services. A visitor must provide contact information (such as name, e-mail, and shipping address) and financial information (such as credit card number, expiration date and billing address) to our credit card payment service provider, CyberSource, for product and service purchases. This information is used for billing purposes and to fill customer’s orders. If we have trouble processing an order, the information is used to contact the visitor. American CRNA Magazine DOES NOT store financial information or credit card numbers. Payment information is managed by CyberSource and Bank of America.

Information Use

The information collected is used to fulfill a visitor’s request for customer service, support, products, programs, and other services, to respond to inquiries about our offerings, and to offer other products, programs or services that we believe may be of interest to the visitor.

In addition, American CRNA Magazine uses the information a member provides to keep our database current with contact information, preferences in communication, and participation in the American CRNA Magazine online community.

Cookies/User Sessions

American CRNA Magazine uses cookies to establish user sessions on the AmericanCRNA.com Web site. A user session is established when a user logs in to the American CRNA Magazine site. User sessions are established to manage access to and within the site. Cookies established within the AmericanCRNA.com user session do not contain sensitive or personal information.

Log Files

Like most standard Web site servers we use log files. Our web hosting provider, Bottomline Web Hosting, tracks the following technical information in our site logfiles in relation to site access: date, time, client ip address, username, service name, server name, server port, server ip-address, method, bytes sent/received, cookies, and user Agent. If needed, we will utilize information contained in the log files to help us in resolving technical issues or general management of the site, as deemed necessary by American CRNA Magazine. This would also include protecting AmericanCRNA.com from unauthorized access and/or use of the site. American CRNA Magazine also utilizes AddFreeStats.com to track visits to our website for traffic reporting and site management.

Communications from American CRNA Magazine

We send all new registered visitors an email with information regarding their registration, once the user has been approved. As a registered user, you consent to receiving email communications from American CRNA Magazine regarding special offers, services, and information regarding your account.
Registered users who wish to opt-out of all communications from American CRNA Magazine, EXCEPT required communication about your account, please contact our Customer Service Department.

Newsletter

The American CRNA Magazine CRNA e-Newsletter is optional and can be requested or declined by registered users via the CRNA profile update in the CRNA Member Area.

Service Announcements

On occasion, it is necessary to send out a strictly service-related announcement. For instance, if our service is temporarily suspended for maintenance, we might send users an e-mail. These communications are not promotional in nature. Generally, site visitors may not opt out of these communications.

Customer Service

We communicate with visitors to provide requested services regarding issues relating to their account via e-mail, phone or mail, in accordance with the visitor’s wishes.

Sharing Information with Business Service Providers

American CRNA Magazine partners with third parties to provide specific services. These services include Web site hosting through Bottomline Web Hosting, and credit card payment services through Bank of America/Cybersource.

As mentioned above, our policy is to share only aggregated data with our business partners and advertisers. However, where such information is required to deliver the service, and where the visitor has agreed to this use of personalized information, we may give business partners or service providers such access to or use of personalized information, as is necessary, for the third party to provide these services.

We do not allow these parties to use personally identifiable information except for the purpose of providing these services. American CRNA Magazine does not sell your personal information to third-parties for marketing, or any other use other than what has been described above.

Legal Disclaimer

Though we attempt to preserve visitor privacy whenever possible, we may be required to disclose personal information when required by law or when we in good faith believe that such action is necessary to comply with judicial proceeding, a court order or legal process served on American CRNA Magazine site, or to protect ourselves, our site or our visitors. Surveys

From time to time, our site requests information from visitors via surveys. Participation in these surveys is completely voluntary and the visitor has a choice whether or not to provide any requested information. Survey information will be used for purposes of monitoring or improving the use of and satisfaction of our site.

List Rentals

We DO NOT make American CRNA Magazine member names, mailing addresses, or other personal information available to other organizations outside of American CRNA Magazine and its service providers. Our member information is not for sale.

American CRNA Magazine maintains practices and procedures designed to ensure the security, integrity and confidentiality of member and subscriber information. Correction/Updating Personal Information

If a registered user’s personally identifiable information changes (such as zip code, phone, e-mail or postal address), it can be updated by the registered user via the user's personal account in the online Member Area. If a visitor no longer desires our service and wishes their profile to be removed from the site, we must receive an email request for removal. Profile removal does not include information saved in previous database backups. Removal of a user's profile does not free the user from responsibility and/or legal liability from the user's use of the site up to and including the date the profile is removed. Profile removal requests must be sent to our Customer Service Department.

Notification of Changes

If American CRNA Magazine implements a change to our privacy policy, we will post those changes to this privacy statement, as well as notify our registered users via email.

In addition, if we would like to use visitors’ personally identifiable information in a manner different from that stated at the time of collection, we will notify visitors via e-mail. Visitors will have a choice as to whether or not we use their information in this different manner.

Children's Online Privacy Protection Act (COPPA)

This notice is hereby given that AmericanCRNA.com will not accept submissions of any kind from children under the age of 13 years. Such unwanted submissions include, but are not limited to, applying for membership to the American CRNA Magazine site as a CRNA, client, or advertiser.

We will not accept communication from children under the age of 13 years via postal mail, email, telephone, or other means of communication. Member registrations found to have been submitted by a minor will be rejected.

Publishing Information/Affiliations

American CRNA Magazine is a private publication. We are not affiliated with the AANA or any other professional organization, public or private. American CRNA Magazine is published as a free private online community for certified registered nurse anesthetists (CRNAs) living and working in the United States.
